When you first attend the practice, you will be asked to sign a confidentiality and release of information agreement. The aim of this agreement is to provide protection of your personal information and to specify who the practice can liaise with in the optimization of your care. Communication between parents in separated or divorced families can be difficult. It is the default position of this practice that both parents should be involved in the care of their child unless there are clear reasons not to, eg Family Court orders, restraining orders, child protection concerns.
